To participate in the MyHabitExperience Survey Sweepstakes, participants must meet the following eligibility conditions:
Open to legal residents of the United States, including Washington, D.C.
Participants must be 18 years of age or older at the time of entry
The sweepstakes is void outside the U.S., including Puerto Rico, Guam, and the U.S. Virgin Islands
Employees of The Habit Restaurants, LLC, the sweepstakes administrator, affiliated companies, promotion partners, and their immediate family or household members are not eligible

How To Take Online & Mail
Step 1:
Visit any participating Habit Burger & Grill restaurant and make a purchase.
Step 2:
Save your purchase receipt. You will need the Store Number and Survey Code printed near the bottom of the receipt.
Enter the required receipt details exactly as printed (for example: Store Number and the 8-digit Survey Code). Incorrect details may prevent access to the survey.
Step 5:
Click the Start button to begin the customer feedback survey.
Step 6:
Answer all survey questions honestly based on your recent dining experience.
Your responses will not affect your chances of winning.
Step 7:
Once the survey is complete, provide your basic contact information in the sweepstakes entry form.
Step 8:
Submit your entry form successfully to receive one (1) entry into the sweepstakes for the current survey period.

You can enter without buying anything or completing the survey.
